Family Dental Care NC - Notice of Non-Discrimination

Complaint Procedure

If you believe that Family Dental Care NC has failed to provide these services or discriminated in another way based on race, color, national origin, age, disability, or sex, you can file a grievance.

You can file a grievance in person, by mail, fax, or email. If you need help filing a grievance, a staff member is available to help you.
